Are you afraid of pretty much everything coming down the road, having physical discomfort (whether true or anticipated), or just living nervously due to fear of the unknown? If this is you, you may have one more thing to worry about: Getting older sooner!
The devil is in the details, or at least, in the telomere. This most important of guardians, the telomere is a protective coating at the end of each chromosome, which protects the chromosome from unusual damage.
Chromosomes contain necessary information for replication (even though we stay who we are, our cells need to replicate, hopefully in the manner originally intended) and without the protective coverings of the telomeres, the information could be lost due to extreme degradation of the chromosomes. The length of the telomeres (the protective covering on the end of the chromosomes) helps to determine the degree of protection of the actual chromosome, in other words, how much of the original material will be protected. The original material was meant to be, we want it protected.
OK, the point of the story has arrived. Phobias, worry, anxiety, any term related to these conditions, create circumstances in which the telomeres become shortened. They become worn down by this “stress” created from within its housing entity (you). The shorter the telomere, basically, the quicker you will age!!!

Politics is stressful. This is only 7 years of calendar aging!

Cancer, cognitive disease, Alzheimer’s, and cardiovascular disease can cause this to happen too, but the mental aspect is much more treatable. With motivation it may be treatable with only minimal outside help. Early identification and treatment are necessary.
For those suffering from forms of anxiety, fear, or nervousness, I strongly suggest the book, or audio series “Psychocybernetics” originally written in 1960 by an incredibly observant and thoughtful plastic surgeon, Maxwell Maltz. There is a rewritten version, done in 2001 by Dan Kennedy, which is also good, but I suggest getting the original book if you can find it. You can find it either on Amazon, or in some Barnes and Noble stores. This book, or audio, could literally change your life, for the better. Hopefully after reading this book, or listening to the audio version, you will see the value in “Chilling Out” rather than “Burning Out”.

Are Energy Drinks Safe?

Recently in a local Palm Beach paper, pharmacist Anthony S. Alferi detailed some detriments of the ever-increasing in popularity energy drinks. You see them in ads on TV, gas stations, even lined up by the “one last item” racks in your local supermarket or Walmart. They appear to be spreading like some super bacteria and may be just as harmful.
Some brand names you may recognize – Red Bull, Rockstar, Monster, 5-Hour Energy, Bawls, Adrenaline Rush, Celsius, Spark, and Slam. These drinks all claim to increase your energy, heighten awareness, and enhance performance. One drink, Celcius, even claims to be a “negative calorie” drink, one that will take more calories to consume than it provides. The instructions tell you to drink it, then go exercise. I’m not sure if that counts as the negative calorie part.
So what’s the problem with these drinks? Since we don’t appear to be a nation of moderation, the sexy advertising and hyped promises of these drinks lead the imbibers to drink in excess, thinking that if a little is good, a lot is better. Most ads even give the subtle encouragement to “guzzle” the can down. These drinks are currently fashionable so it is common place to see them in schools. Whereas 15 or 20 years ago sneaking out and smoking with your friends was cool, now it’s the energy drinks. However these drinks can cause harm much faster than smoking.
There’s another problem that the drink manufacturers don’t want to discuss. Some young people, in their infinite quest for the next level, mix energy drinks with alcohol. The fast high you get can make you feel downright unstoppable. Assuming you don’t end up in the emergency room. Also, binge drinking associated with energy drink cocktails can be even more “fun” because you get a fast high without the side effects of fatigue and drowsiness you get with alcohol. Until you come down from that high and find your fatigue level crashing. Until you get your hands on another energy drink to pick you up.
In his article, Mr. Alferi told about a 14-year-old schoolgirl who went to the mall with her friends and consumed Monster energy drinks. A few days later she was dead. The cause of death was ruled to be “heart arrhythmia due to caffeine toxicity.” Medical articles have reported effects of energy drinks, specifically Red Bull, to be increased blood pressure, arrhythmias, and tachycardia (rapid heart rate). These drinks have caused seizures in otherwise healthy individuals. In other articles neurological toxicity, palpitations, strokes, and death have been reported. Can it really be worth it?
In October, the National Federation of State High School Associations cautioned that caffeinated energy drinks – often confused with such products as Gatorade, a fluid replacement drink – should not be consumed before, during or after physical activity because they could raise the risk of dehydration and increase the chance of potentially fatal heat illnesses. The organization also warned of possible interactions with prescription medications – including stimulants used to treat ADHD, or attention-deficit hyperactivity disorder.

I will admit to enjoying a cup of coffee or two in the morning, but being prone to palpitations and rapid heart rate already, the increased caffeine combined with other ingredients scare me to death. I can’t comment on how great they may taste but from looking at their marketing strategy it can be assumed that the experience after consuming is far more important than taste.
As a final warning, I mentioned earlier some are also being marketed as “weight loss” drinks. This can be a dangerous game for an adolescent whose body is overweight and as such, already experiencing increased coronary risk.
My advice, and the advice of most medical professionals, is to stay away from these drinks, and definitely talk to your kids about the dangers.

Counterfeit Drugs Out Of Control

Most of us have some type of prescription medicine in our cabinet. It may be something simple to relieve a temporary difficulty, or medication to help us through a long term problem. In either instance, we are depending on that drug.

In February the FDA found counterfeit Avastin, an injectable cancer treatment, had been distributed to 19 cancer treatment centers in the United States. The centers all purchased drugs that were in short supply from online distributors here in the United States that were buying from foreign suppliers. Unfortunately, those receiving this counterfeit drug got none of the key ingredients they needed. We will never know how negatively this affected the recipients of the fake drug.

While we are all aware that counterfeit purses, music Cd’s and even jewelry is being brought into our country, did you know that counterfeit drugs can be 100 times as profitable? Especially if there is a shortage of a particular life-saving drug.

In May, a man right here in Palm Beach was convicted of re-labeling and packaging Chinese test kits for sexually transmitted diseases including HIV. The kits contained nothing to prove or disprove the diseases. Naturally they also had no FDA approval as his web site claimed. How many people were affected by his greed?

In May again, the FDA found counterfeit Adderall 30 milligram tablets. Adderall is used to treat ADHD and narcolepsy (sleep disorder). The pills contained large amounts of pain medication, but naturally no ingredients for the intended purpose. You don’t need to be a medical professional to realize the tragic consequences that can result from the wrong medication. The Adderall situation is is so recent I will simply copy an excerpt from the FDA press release for you:

“The FDA’s preliminary laboratory tests revealed that the counterfeit version of Teva’s Adderall 30 mg tablets contained the wrong active ingredients. Adderall contains four active ingredients – dextroamphetamine saccharate, amphetamine aspartate, dextroamphetamine sulfate, and amphetamine sulfate. Instead of these active ingredients, the counterfeit product contained tramadol and acetaminophen, which are ingredients in medicines used to treat acute pain.

Currently on the FDA’s drug shortage list, Adderall is in short supply due to active pharmaceutical ingredient supply issues. Teva continues to release product as it becomes available. Consumers should be extra cautious when buying their medicines from online sources.  Rogue websites and distributors may especially target medicines in short supply for counterfeiting”.

Here’s the bottom line. We all need to be acutely aware that buying drugs from online sources is not safe. Even my vet has a warning in his office about buying pet medication over the internet.

On the positive side, as far as I know, no prescription drug has surfaced in conventional sources, such as a licensed and trusted pharmacy.

The counterfeits are coming from online, out of state, or mail order sources.  Stay away from these operations. You may save money, but what problems are you buying in exchange?

How Observant Are You?

Observing someone you admire, then modeling the way they behave in different situations is a great way to improve yourself. Many great leaders give credit for their success to someone they observed in leadership before them.  If they were not able to meet that person, they literally studied them from a distance.

Watching how someone dresses, what gestures they use, how they talk, and what words they choose to express themselves can be an education for the willing student. Could you benefit from taking the best qualities of someone you admire then integrating their habits into your personality?

Naturally the answer yes, everyone could benefit from a mentor they admire. Even one you are unable to meet personally.  But, first you must be observant. How are your powers of observation?

Since many times I need to pick up on subtle signs from patients about their concerns, I thought I was pretty observant.

Being Beautiful In This Economy

There are a huge amount of issues and feelings going on with this poor economy. Especially when it directly affects you. Have you had to adapt, give up, throw away, keep, or just plain forget about something because of what’s happening today? So many of us are caught up in the last few years of prosperity we’ve forgotten what it meant to feel great, because there is so much gloom and doom.

Branding Your Own Identity

Do you have a certain “style” in your outfits that fits your individual personality? I call it your Brand. Advertisers are notorious for appealing to consumers by creating an image, an identity, something that causes the “it” factor to shine so that you not only recognize it, but can relate to it or buy it.

People are like that too. We all have our own brand- some good, some bad. For instance, if you spend all your time in sweatpants (and you never work out), a T-Shirt, your hair pulled back in a ponytail and no makeup- then that’s how people see you. Whether that is a good thing or a bad thing is your perception- but I’d like to have a little more personality shining through than merely “au-natural”. Why? Because your brand in this case may cause others to see you as apathetic and lazy. I want them to see someone who appears confident, feels fabulous, and exudes a unique character!

Many people had never heard of Kate Middleton, the Princess to Prince William before they married. But now every time you open up a celebrity magazine you’ll see her, her clothes, what hat she’s sporting, and the jewelry she’s donning. Much ado has been made about her fashion sense, and now others are mimicking her style she so easily displays. And her closest confidant’s will tell you that she has always carried off this pristine style (think again, BRAND). Obviously, we can’t all afford to brand ourselves as a Princess, but we can discover our own inner personality and make that apparent.

How do you do that? First, choose an outfit that shows your brand. One of my patients always wears light, flowing white clothing…usually a beautiful linen blend. No matter how hot, sticky, cloudy or wet the weather is outside, she always creates an air of invigorating freshness whenever she walks in. She is simply crisp, cool and lovely! Wow- wouldn’t you like people to say that about you when you enter a room?

It can happen. Go find a color and an outfit you feel great in. One that makes you happy, gives your skin a natural glow, and is comfortable. It doesn’t have to be bright turquoise. Some people look and feel great in Black. It’s your identity- no one else’s. Wear what makes you feel and look positive.

Once you’re dressed, look at how you carry yourself. Your posture speaks volumes. Walk with your back straight while resting slighting on your back hips. This screams self-confidence. Practice this at home until it becomes second-nature. Avoid walking slumped over with rounded shoulders and head down. This gait has the opposite effect; reeking of desperation, worry and low self-esteem. Chin up- Back Straight- Chest out- I know, I sound like a drill sergeant, but you only need to look at a Marine to know what I’m talking about when I say -walk the talk!

Another thing to look at besides clothing and posture is your hair. Hot muggy weather is a problem in South Florida. Frizziness has been the downfall of many beautiful hairstyles that looked great in front of the vanity only to curl and kink up after a few minutes in the heat. If heat and humidity are part of your everyday life, it may be time for a Brazilian Keratin treatment. (No, I didn’t say Brazilian Wax- we’re not talking about waxing your head so you look like a shaved Brittany Spears). A Brazilian keratin treatment takes an hour and a half but leaves your hair smooth and silky. It takes the guesswork out of how to control that hair. And now, with all the advancements in hair care, the treatments don’t have the awful smell that used to be associated with them. You’ll end up with luxurious, manageable, frizz-free hair- no matter what the weather’s like!

So what are you waiting for? Let’s start impressing them with a new enhanced image- discover and promote your Brand! New colors, better posture and fabulous hair- you could have a whole new persona in no time. Then you really will enjoy your weekend plans!

Deal or No Deal? — Cosmetic Dermatology Costs

Many factors determine the price of cosmetic dermatology procedures. Obviously we all want to keep our costs down when paying for injectable services- but careful consideration must be taken! Let’s be honest- we are talking about your face here. Generally if you pay for a KIA, you’ll get a car that looks, feels and drives like a Kia, not a Mercedes. Cosmetic Procedures aren’t much different. You get what you pay for and this is evident in not only the product itself, but also the provider, the overall office experience, and most importantly- the results you see in the mirror.

Now with that being said, it doesn’t mean that the most expensive prices net you the best service. So what’s a consumer to do? I simply suggest asking. There are Bargains to be found.

Because of increasing competition between vendors like Medicis and Allergan, deals are being offered. Also, due to today’s economy, plastic surgery services have been hard hit. This downturn in operative procedures has made injectible services more important in offsetting a Dr.’s losses in plastic surgery revenue. Some suppliers are partnering with offices to offer consumer incentives. My office does this frequently. Other times, an establishment may offer lower prices to simply undercut the competition. We never do this! If a practitioner wants to know what someone else is charging, and will base their price to you on that figure, RUN. They are desperate and care far more about their bank account than they do about how you are going to look.

So, how do you know what to do to get a fair price? Ask. Ask. Ask. Ask your friends who they have confidence in. Ask your family where they go to for beauty services. Then call those places and ask them if they have any specials going on. Some offices that do higher volume may get rebates from the vendors for selling their products. In many cases these rebates will be passed along to the patient, resulting in better discounts for you. Ask if that’s a possibility whenever you call regarding the procedure you’re wanting.
If you walk into an office loaded with information, you’ll be able to determine if the price you’re being quoted is a fair, and sometimes even Great deal. As long as you know what questions to ask about your procedure before you pull out your pocketbook, you can feel comfortable and safe in knowing you’ve picked the right technician and the right office.

Note- Beware of any receptionist who won’t give you a rate over the phone. If they tell you that you should come in to get a price, look elsewhere. Every reputable office should have published prices somewhere. And if they really have a special to give you, shouldn’t they be able to tell you what it is? They should at least be able to give you the discount percentage over their regular rates in a telephone conversation? Why should you have to waste your gas and time to go to their office just to discover what they charge?

Lastly, if you see prices from an unfamiliar or brand new office posted that simply look too good to be true, it would probably be smarter to stay away. We all read in the papers how medical locations are closed for operating without a license, using untrained staff, or (what’s even scarier) administering the wrong product- simply to save a buck. Don’t be one of those who find out from a news report. Stick with who you trust. This is your face after all. And some deals are just no deal at all!

Celebrities Get Paid To Endorse Products

I know you realize that advertisers who use a celebrity, do so to gain product fame and notoriety. This tactic has been around for years- ever since marketers realized that the average consumer trusts and respects certain famous people. If you buy a product merely because you believe the spokesperson, then you have proven their point.

Remember how at one time everyone wanted Tiger Woods to endorse their product. It was like, if Tiger uses it, it must be great. But as soon as people read negative things about him in the paper, advertisers started dropping him from their campaigns. They felt his endorsements might actually hurt their sales if people aligned the image of their product up to the image of the Tiger Woods.

So what’s a consumer to believe? I mean seriously, did the leaders at Pepsi really believe that Gatorade drinkers would stop chugging Gatorade’s Tiger Focus drink because he was unfaithful?

I honestly don’t know what they thought privately, but I know he lost endorsements from some huge companies. The point is- advertising needs to be more honest. But- that is unrealistic. Great idea, but not very likely to happen overnight.

Don’t expect total transparency. It will never happen. Advertisers spend too much money to simply put the facts out there and expect you to make an informed decision as to whether to purchase or not. That would be too easy, right? But changes are coming.

A new regulation proposed by the Federal Trade Commission (FTC) will significantly affect the way advertisers and endorsers are allowed to operate. Once this is adopted, you can expect many subtle differences. According to Andrew Lustigman, a New York Attorney who represents direct marketers, media and internet companies, and advertisers as a whole, these changes are to be in effect to give further protection to the consumer from misleading testimonials and endorsements.

What that means in a nutshell is you may start hearing more disclaimers at the end of ads. They’ll be forced to report and describe typical results seen. Right now they’ve been allowed to show the most extreme examples of success and then flash words across a screen saying, “Results may vary” or have an announcer talk so fast that you can’t understand his disclaimer. Now they’ll be required to truly reveal a typical result. And typical will mean you, the average user, can expect to see.

The bottom line is expressed in the words “evidence based”. In other words, don’t just show me a photo of someone who looks better, show me the results from the study. And, the study needs to represent a cross section large enough to be meaningful and be performed by an independant source. That’s important.
Personally I’m all for more honesty in advertising- especially in skin care products – even it means I won’t be seeing as many of my favorite celebrities anymore.

Beauty and Your Pocketbook With Botox, Sculptra & More

I advise all clients to shop around when they’re trying to choose a provider to do their beauty procedures. Though you’re not exactly car-shopping, many of the same techniques to help you decide should be used. You are looking for comfort, safety, performance and price.

One big exception to that analogy is that we are talking about your face here, so when asking about products, realize that you’re also paying for the security of knowing the medical provider is skilled, the product will be able to provide you with what result you’re looking for, and that the price is something you can accept.

So, with that said- what are some average prices for the most commonly used beauty enhancements? Unfortunately this is a tough question because it varies so much. Lets deal with the most popular treatment first, Botox. In some cases, Botox can vary between $150 dollars to $1,200. This is an extreme difference! Why would one office charge $200 for Botox while another bills you $1200? That’s the type of question that will arise when you do internet searches or even call around in your area for price points. They rarely give you any details, just “special prices.” So what are you to do?

Botox 
First look at the unit cost. If you ask the office for a unit price, they should be able to tell you. And if you know the unit price and have had the procedure before, you can estimate the total by multiplying that unit cost (say 15 dollars a unit) by the number of units used previously. But that wont help if they charge by the area. Some offices charge by facial areas. Many will say “3 areas covered” in the total price. So if you have a frown line between your eyes, that would be one area, and your crow’s feet (naturally one on each side) would be two more, for a total of 3 areas treated.

Here is a little quick advice: NEVER shop for the cheapest price point then show up for your appointment prepared to only pay that much. Remember, many times if something sounds too good to be true- as in $150 dollars for a Botox treatment, it probably isnt. Many times when you get in the office, the rate changes or suddenly there was a “misunderstanding”. Don’t be so intimidated you just stay. Leave and leave fast.

The new kid on the block in muscle relaxers is Dysport. And it’s an excellent product. Unfortunately Dysport is measured in a different manner, so the advice above regarding units only pertains to the Botox product line. The “by area” price with still hold true with either of them though.
Now lets look at some popular filler guidelines for you to use in your search-

Restylane and Juvederm
You may have seen advertised specials for Restylane and Juvederm ranging between 300 dollars to 700 dollars. Both of these hyaluronic acids will be great fillers for you, but beware if the price touted drops below 300 dollars. Many times a special in this price point has hidden in fine print it is only for a half-syringe. That amount won’t do much unless your needs are for very, very subtle changes. With that said the average price you should expect to see for these products are between $500 and $600 dollars. (It could be a bit more if you’re receiving Perlane or Juvederm Plus because they are larger molecule products).

As I’ve said many times, dont be taken in by the highest prices, but be even warier of the lowest…many times these offices are desperate. An office producing great results with qualified medical providers will never be at a loss for customers!

Sculptra
Sculptra, produced by Sanofi-Aventis, has been around since 2004. Though the cost has decreased slightly in the last 7 years, it is still pricey. Sculptra is sold by the vial (or occasionally by the cc) and the average price is $750 to $900, but I’ve seen it hit the $1500 range as well. The most important thing to remember with this procedure is the competence of the practitioner. Make sure they are experienced and an expert at these injections. In the right hands, this treatment is absolutely phenomenal. The results are not immediate, but they are impressive as they begin to take effect and transform your face making you look younger and more refreshed.

Radiesse
Radiesse is another product released in 2004. Back then, you really didn’t see it for less than $900, but now this robust calcium-based product is sometimes advertised for as little as $500. If you want immediate-effect sculpting, this is a super option. By comparison, a syringe of Radiesse contains 1.55 cc in contrast with the 1 cc you’ll receive in Restylane and .8cc of Juvederm amounts. Though the top of the line Radiesse can still demand $900 dollars, you’ll be amazed at the difference in your appearance. I think you’ll find it is well worth the expense.

In conclusion, whatever you do, don’t shop simply for price. Research the practitioner and their experience; the office and its volume of clients; product turnover (you don’t want anything that’s expired, do you?) and the reputation of the facility itself. In this, as in almost every purchase you make, you ususally get what you pay for. And a good reputation is worth a few extra bucks!

Balance Is The Key To Maintaining Your Weight

It seems there is always an excuse to over-indulge- a continual barrage of enticements all year long. During October, all the stores are stocked full of candy for Halloween. During Christmas, you’ll enjoy delicious holiday meals, desserts, and parties. Shortly after that comes New Year’s Eve celebrations followed by Super Bowl parties! Next rolls in Valentine’s Day with tons of chocolate confections and if you’re lucky, a romantic dinner out. About the time you have recovered from that 5 month onslaught, it’s Easter and more family get- fun serving up huge turkeys, glazed hams and sweet potato casseroles. The list goes on and on; so by summer time, we’ve added pounds to our scales and inches to our waistlines.

I’m often asked how to lose weight. People want to be thinner by summer so that when bathing suit season rolls around they can walk confidently down the beach. The truth is- you must maintain and prevent all year long. It is definitely harder to lose weight during the festive months when gorging, heavy drinking, and partying seem to be the norm, but you must plan ahead. Don’t worry, I’m not telling you to avoid food and alcohol, just don’t blimp out! Indulge a little- enjoy the treats and delicious home-made foods. That’s part of what makes the seasonal holidays so special. But be aware of how much is too much and put limits on yourself.

If you decide that you really want that slice of cheesecake, or that second helping of beef tenderloin- then go ahead. But set out to do something to offset it. Like the dreaded word Exercise. I realize it can evoke locker room horror stories if you weren’t the athletic type growing up, but it really can be quite enjoyable. Take a nice jog through your neighborhood. Instead of just walking your dog, try a steady trot. Maybe a bike ride is more your style. In Florida, swimming is almost a year-round option, and a terrific way to stay fit and trim. Many of my friends break down and join a gym. The camaraderie (and gossip) that can be heard on those treadmills is worth the membership fees alone.

The great thing about exercising is you can still indulge but do so without all the guilt or worry. You may find your weight isn’t merely maintained, but actually decreasing. I’ve also learned that if you get it out of the way early, you’ll be more likely to complete your goals and enjoy the rest of the week. The added physical activity can even pull you out of your usual morning sluggishness and spur on those endorphins to really jump-start your day!
Give it a try. That way you can eat without guilt, and still look great walking down the beach or the aisle!

Brand New week means a Brand New You!

Lots of people hate Mondays. But I look at them as a new beginning; a new week, full of fresh possibilities. Start your week off right by imagining a brand new You. Have you discovered your personal Brand yet? What I call your “it” factor, the one thing that people connect with when they see you. Maybe it’s your humor, your confidence, your ability to be unflappable. Whatever it may be, you should be able to identify it, and own it. But don’t worry if you haven’t discovered it yet, lets work on that together.

So, let’s get started. First, decide what your brand will look like today. In a previous article, we talked about branding for confidence and success. Now we need to fine-tune the concept of who you are, or who you want to be.Pick one thing to work on each week. Just one. Once you’ve decided on that one thing, focus on how you’d like to add to it, or change it, eliminate it, or expand upon it.

Here are a few examples of what I’m talking about-
Maybe you want to be a person who is always punctual. Then work on that all this week. Be conscious of your time, your appointments, the length of your conversations, your commute time, and anything and everything that could keep you from being precise and on time. You’ll be amazed at how a simple thing like time management and proper planning can change your whole week!

Another goal might be to feel prepared for anything! Maybe the last month has thrown you one curve ball after another until you simply feel like if one more thing goes wrong, you’ll crack up and lose it. Don’t let it get that far. Instead, try to imagine all the ways that you can counter an attack head-on. Think up possible “what-if” scenarios and rehearse the different options in your head. If you already have thought about how you’d respond, and the outcome you’d want, then you’ll be less likely to have it backfire. The way you react can totally transform the outcome.

I had a patient do this when she thought her child may be diagnosed with a serious health ailment. She played out all the different scenarios in her head, was able to logically think of questions to ask the doctor, and more importantly was able to calmly look at all the facts from an objective, unclouded place in her mind. That way, when she did walk into the office, she was mentally prepared for whatever outcome he laid on her. She was able to listen and hear him, instead of just answering out of fear and spontaneity. This is what I mean by taking charge of your mind.

You can never control the action- only the reaction, which is your response.
So what one thing do you want to work on this week? Maybe it’s getting back to exercising, losing that last 10 pounds, taking that computer class you’ve been talking about, or finally making amends with an old friend. Whatever it is…you want to choose the one thing that is your most pressing desire.

Don’t worry if you have more than one- we all do- but the point of this is to pick one each week and focus on it. Put this goal on the dashboard of your life. Don’t worry about the others, just like next Monday; they’ll still be there to work on.

So what are you waiting for? (I hope your dream to stop procrastinating hasn’t already failed). Seriously, start now. Set the desires of your heart afloat, and then reel it in and day by day you’ll be able to confront those challenges that have seemed to be just out of reach. Repeat this week after week, and soon you’ll discover a brand new you. Remember, practice makes perfect! A perfect, beautiful you, right now!

It’s often been said that beauty is in the eye of the beholder.
That may be true, but more recently, it is also in the eyes of the bank holder. As in who holds the money. There are many options out there to achieve a younger fresher look-ways to diminish wrinkles and literally reverse years off your appearance- but as we all know, it comes with a price.
So make sure you do the research needed before subjecting your body and your pocketbook to potential damage.

Assess where you are now. Honestly evaluate your age, your skin damage (age spots, melasma, chelitis), the wrinkles, furrows, crow’s feet, dark circles and sagging skin you may have. Do you just have one thing you’re trying to correct? Or do you desire a complete overhaul? Once you’ve decided what you would like to achieve, seek out several sources which can accommodate your treatment plan. By contacting several sources you’ll be able to get varying opinions as well as a timeline and price guide from each provider. I call this To-Do list your treatment plan because it’s rare to have everything you want accomplished in one visit. Obviously if you just need a little Botox or Dysport to remove a brow crease, that’s an easy fix and one visit can do it. Just Because It’s Natural Doesn’t Make It Healthy
Don’t be surprised if while looking for options you discover operations run by general practitioners, chiropractors and therapists who want to offer you a “holistic” therapy. In addition to feeding you the wonders of basic supplements, they may tout you should have holistic cleansing and toxic reducing therapies. Many times their approach at all-natural ways geared to keep you young are far from legit. I’m not saying all of these holistic practitioners are dishonest, but I do advise you to use great caution when considering their claims. Just remember this- Just because things are all natural doesn’t necessarily mean its all good for you. Arsenic is all natural but you don’t want to drink it. Poison ivy is too, but I wouldn’t want to rub it on my skin.

In another article I write about the average costs of treatments that are designed to preserve or revive your youth. This will give you a base with which to work from, but keep in mind that it’s always a great idea to get several quotes. Just keep in mind the single most important criteria you must use to make a choice is the practitioner you will use. That choice is far more important than price.

There is a woman known in South Florida as the Botox®-Bandit. Unfortunately, this title was earned. She made a habit of going into Spas and medical offices, receiving Botox® injections and then skipping out on the bill. I just wonder who she is going to impress in prison with her wrinkle-free face.

Seriously, I know that times are tough, and beauty enhancements may seem like the last thing you can afford in this economy. People are sacrificing their anti-aging treatments because they think they can’t afford it. But the truth is, where there’s a will there’s a way. Let me show you the honest way because no one needs to stoop to being a Botox® Bandit to stay beautiful.

Even on a budget, you can receive top quality service at a discounted price. Many offices will offer a reduction off the final bill if you combine it with something else. Rarely is this published; but it is frequently honored if you merely ask. Do not hesitate to request it. For instance, let’s say you go into a high-end office for a few procedures and they charge you the published price for Restylane®, Botox®, and some Radiesse®. My advice would be for you to say, “Since I’ve purchased so many treatments at one time, aren’t I entitled to a discount?” Nine times out of ten, you will get it. I know we keep an open line of communication about pricing with my patients. I appreciate them and want to keep them happy.

With so many providers out there to choose from, it’s important to us as professionals to keep our clientele happy. If we don’t, we shouldn’t be surprised if they search for someone else a block away.

Mondays must be the most despised day of the week. It really gets no respect! Most people with a 9-5 five day work week look at Mondays as something they simply have to endure and get through. Even the popular motto- “I don’t do Mondays” shows how many try to ignore it, bypass it, or suffer through it at best.

But I’d like you to consider Monday a blessing instead of a curse. Even that blasted alarm clock that rings you awake should be met with enthusiasm, not dread.

Since perception is everything, start viewing Mondays as a chance for a new start instead of that dreaded woe is me feeling. By making this mind-change, you may actually begin to welcome Mondays on your calendar.

By doing a few subtle changes, like getting up 15 minutes earlier on Monday mornings and going out to dinner on Monday nights, you may find yourself looking forward to a new week.

As far as your work schedule, in the past, maybe you put off jobs last week and now you are faced with them first thing Monday morning. What if you decided to stop putting off all those things and freed up Monday for new beginnings? By accomplishing those nasty tasks the week before, you can plan a bit on Monday and breathe easier the rest of the week knowing things are more under control.

Another thing I’ve learned is the power of positive reinforcement. Let’s face it, most people tend to be happier and more pleasant on Fridays because they know the weekend is around the corner. But what if you switched things up a bit. Instead of being depressed and complaining like the general public does on a typical Monday, you are instead extra nice. Think of how much you will stand out from the pack of groaners if you are the happiest one in the room. Just an extra smile, or a touch of kindness may be all thats required to do this.

By taking charge of Mondays and regrouping- you will begin to change your whole week- for the better! Give it a try, you’ve got nothing to lose and everything to gain!

Look Younger On A Budget

One thing I’ve noticed lately is how tired, angry or worried today’s working woman appears. Though I realize times are tougher than ever, it’s important to not look worn out, and thats what I see. I suggest investing in a little Botox. A little goes a long way and when done well, and no one can guess you had it done. Your face, especially those furrowed brows and crinkled crow’s feet, will appear relaxed and smooth. Where co-workers once saw a stressed-out, worried woman, they’ll now believe you’re relaxed and totally in control. Something you definitely want to portray before you go ask for that raise!

Another tell-tell sign easily fixed but often ignored are gray roots. We have all seen the “line of demarcation” when the color has grown out, and our real color-(washed out gray)- comes shining through. I also meet many women who are tired of living with old sun damage. I see more than most doctors do because I live in South Florida. Most of my patients are pleasantly surprised to discover there are relatively easy fixes to the uneven tone and multiple brown spots that have popped up on their face. Going to a cosmetic medical provider and getting some IPL (intense pulsed light) and some Retin-A or other retinoid will go a long way in reversing the sun damage and improving your skin tone. The retinoids actually make your skin look younger, not just more even!

Magic Skin Creams. Fact or Fiction?

Every cream seems to claim you will look 20 years younger….If you just use their product!

The infomercials, department store sales ladies, and drugstore shelves, all make unrealistic promises.
One big reason their promises are never kept is because the “key ingredients” contained in these lotions and creams are simply not enough to make a real difference. Every jar they sell has to be formulated for the person who’s skin is the most hyper-sensitive. If not, they would be sued by the first person who’s skin was damaged with their product. That means you may end up using a solution that contains only a tenth of the key ingredient you need to make a difference for your type of skin.
But don’t lose hope. There is good news. Retinoids, anti-oxidants and even coffee berry products in the hands of a trained practitioner will produce more beautiful youthful looking skin right now.
Retinoids:
Your first line of defense should be to use a retinoid. Unless you’re planning on getting pregnant, a retinoid will make your cells turn over faster. The faster these cells reproduce, the more healthy and better your skin will appear. This reproduction actually increases nutrition below the top layer of skin and helps prevent pre-cancers. Some Retinoids are marketed as Retin-A®, Diferin®, Tazorack® and Obagi®. These products will thicken your skin, and actually cause wrinkle reduction.

The Acids:
The next product you should use is an alpha hydroxy acid. My favorite is Glycolic Acid because its got the smallest molecule and is able to penetrate the deepest. Gycolic Acid grows more of the dermis and increases the thickness like the retinoids do. There are many acids to choose from- Citric acid, malic acid, manelic acid, lactic acid, tarteric acid…and the list goes on and on…but the best is Gylcolic! All these are made to get past the stratum corneum- that’s the top layer of dead cells. Once it penetrates this top layer, the real healing takes place, and your skin elasticity will increase.

Unfortunately, many of the creams that claim to fight aging aren’t really getting below the stratum corneum. They simply bloat the dead cells with moisture, making any fine line wrinkle look like it’s less. But it only lasts while it’s on there. It comes back the first day without the goop.
Anti Oxidant & Free Radicals:
Free Radicals & removing them with Anti Oxidants is important. Free Radicals are attacking us from every direction- From the environment, from smoking, the sun, the air, our metabolism… you name it. We need to combat that with Anti-oxidants. Some common ones include acevedo and coffee berry extract.

Summary:
I promise if you use a combination of retinoids, glycolic acid and anti-oxidants, you will begin to see major improvements in the appearance of your skin. By taking off dead cells, thereby replenishing and thickening the new dermis cells underneath, and fighting those free radicals, you’ll be looking beautiful right now in no time at all!

Confidence and Self Esteem Aren’t Economic Terms!

Many people would be surprised at what a patient reveals to her doctor. I may not know as much as their hairdresser, but I hear some secrets. And in today’s economy many of my patients are concerned. Very concerned. Though the income brackets of my clients vary greatly; the stories I hear don’t. I have found it doesn’t really matter, income wise, if you are a working family trying to get by on less income or a Palm Beach mansion owner having to sell hard earned real estate assets at fire sale prices, you face the same thing. Only the zeros on the end of the numbers change. So when a client has to decide whether to pay their mortgage, go out to a fancy restaurant, or skip a Botox injection, you may be shocked at their decision.

I don’t know of any who have skipped their house payment, but I do know that dinners out have been traded to find that “extra cash” to keep themselves looking fresh.

If you’re searching for a job, it’s no secret that you need an edge to get the job over other applicants. Everyone who goes to an interview should be wearing their best business attire, understand the company’s philosophy and have done their homework for the Q and A. But not everyone can pull off the confidence factor. By erasing years off your appearance you will embody the self-confidence needed to seal the deal.

Even if you aren’t job shopping, when I ask my patients what motivates them to keep getting the treatments, they all share a common answer. They need it for their confidence and self-Esteem.

My clients tell me that they simply feel better about themselves when they look better. As one lady said- “Things are bad enough out there. I have to look in the mirror and see something good”. Quite simple, yet eloquently spoken!

Is Bee Venom Better than Botox®?

A new fad has hit the market, recently gaining attention because the Duchess of Cornwall was using it. This product is referred to as the “Bee Venom Mask®”, from Heaven by Deborah Mitchell. This mask is now being touted as “Nature’s Botox®”.

Does it merit consideration, or will you be stung (sorry, I couldn’t help it) by another misleading advertisement?

Since the claims are saying this is “Nature’s Botox®,” lets see why they’re comparing it to Botulinum toxin-type A, which is also found in nature. The words “organic face lift” is how they’re describing the results you’ll see from using this “Magic Bee Venom®.” But, if any of you have ever been stung by a bee, you know that swelling doesn’t necessarily mean beautiful.

The main component in honeybee venom is mellitin. Yes, it is a natural substance which is a strong base alkaline peptide made from honeybees, but this is also the main irritant that causes so many horrible side-effects when one is stung. They are essentially banking on the fact that you will only have the side-effect of swelling, and not one of the other more serious life-threatening emergencies like anaphylaxis.

Bee stings, nor their mellitin, can immobilize muscles like Botox® or any other FDA approved muscle relaxant will. Mellitin merely gives a temporary, almost-confused effect to the tissues it is applied to. This superficial “confusion” can result in some tightening and swelling on the face, but no more than the use of a drying clay mask would. Clay will help pull together tissues for a brief time, but the results are very short-lived. The same is true with the chemical venom from the honeybees.

Unfortunately, many people equate the words “organic” and “natural” as synonymous with healthy and safe. Many buy products because they believe it is more beneficial to use something created naturally than something received from a syringe. Nothing could be further from the truth. This is where I must remind you that just because something is organic, doesn’t mean it’s safe from potentially harmful substances. Take the following examples; all completely natural, but devastatingly harmful if used inappropriately:
Hydrochloric Acid- This is created by your own body, yet can destroy your own tissues.
Arsenic- This is found naturally in metals, but this natural poison interacts with proteins, and is often used in pesticides because it can be lethal.

Hemlock- This is a known poison that grows wild. This comes from the Apiaceous family.
Queen Anne’s Lace- This flower/weed can be found all over the world and can be used as a contraceptive, because it is able to dislodge a newly fertilized egg from its hold. This weed comes from the Daucus Carota family.
Poison Ivy- This natural plants secretes an oil that almost everyone is allergic to. Itching, redness, blistering and oozing are common reactions to those who come into contact with this plant. And if Poison Ivy is cut or burned, the fumes are potentially deadly to animals and plants who breathe it in.
All these are Natural yes, but definitely not good for you!

So, if nothing else, please realize that companies love to make claims of their products being natural, green, organic, or pure. And while this may be true- it does not mean it’s safe.

Botox, and other muscle relaxers, Dysport and Xeomin, have tons of research and testing to gain FDA approval for cosmetic purposes. Bee venom is escaping for now this needed approval, but I am betting that in the future we will hear of many bad reactions and you will see the product fade away and be replaced by the new kid on the block, whatever that may be.

Is PROACTIVE® a miracle system?

PROACTIV has been very sucessful with many celebrities touting its effectiveness. The two Dermatologists who created the Proactiv® System are Katie Rodan and Kathy Fields. These two realized that using an ingredient central to acne care and putting it all together in an easy to follow system would generally produce good results. So, is it a miracle system after all?

Well, the question I’m most commonly asked isn’t whether it works or not, but what exactly are the ingredients in Proactiv® and can you get them elsewhere?

Because let’s face it, the PROACTIV® system isn’t cheap and if there was a way to get the same results for cheaper, wouldn’t you want to know?

So let’s start with a lengthy explanation of what the actual ingredients found in PROACTIV® are. And yes, the good news is- you can get them in many other places beyond this one system.

As you look at this list, you will see many ingredients and wonder why it takes so many, especially the synthetic ones, to achieve the purpose of diminishing acne. Basically the concept is simple- you have a very powerful “working” ingredient in benzoyl peroxide, but you need a vehicle to deliver it to the skin without harming it. In fact, you’d like the vehicle to actually benefit the skin, like nourish and cleanse it while delivering the potent product. I liken it to the one-two-punch before the knockout.

A good marketing concept always wants its product to be somewhat pleasing in texture, appearance and scent. Many of the ingredients found in PROACTIV® are there to simply achieve this. They are used as a filler, to fill up the bottles while promising to leave the skin better than it found it.

So, here’s a technical look at the actual ingredients listed on the PROACTIV® system, starting with #1 in the system- Renewing Cleanser.
The main ingredient is benzoyl peroxide. This is one of the most widely prescribed and used acne treatments available. It is available almost anywhere from an over-the-counter (OTC) dosage to one that requires a prescription. It can be bought in cleansing bars, liquids, gels, creams or lotions. It usually comes in strengths of 2.5%, 5% or 10% strengths. Sometimes it is combined with other products such as antibiotics to provide a combination therapy plan.

The list of other ingredients and their purpose in the Renewing Cleanser are:

Anthemis nobilis flower extract – Roman or English Chamomile, often used for fragrance or skin conditioning; Cetyl esters – synthetic wax used as lubricants; Dimethyl isosorbide – possibly a dimethicone to add smoothness and texture; Disodium dimethicone copolyol sulfosuccinate – Surfactant, cleansing agent; Imidazolidinyl urea – Antimicrobial preservative; Magnesium aluminum silicate – A naturally occurring mineral often used for a thickening agent; Methylparaben – A preservative; Neopentyl glycol dicaprylate – Used as an emollient and thickener; PEG-100 stearate – Made by combining natural oils with Stearic Acid. Used as an emulsifier, emollient, and moisturizer; Propylene glycol – A small organic alcohol used as skin conditioning, fragrance, solvent and humectant. It is associated with a high level of irritation and contact dermatitis; Propylparaben – Fragrance, preservative. The parabens can mimic estrogen and disrupt the hormonal system; Sodium hyaluronate – a synthetic version of one naturally made that promotes lubrication and hydration; Sodium PCA – A humectant moisturizer without additional oils; Sorbitol – A sugar-alcohol that is metabolized slowly by the body; Tridecyl stearate – Emulsifier and thickening agent; Tridecyl trimellitate – A very heavy, odorless liquid that has a syrup consistency, is a bit tacky, but leaves the skin soft; xanthan gum – A thickening agent and a stabilizer; Water – I am hoping we don’t have to expound on this one.

The second bottle in the PROACTIV® system is called Revitalizing Toner. Many people are confused as to the purpose of a toner, and rightly so. First, a toner is supposed to remove any excess dirt after cleansing and to return your skin’s pH to it’s normal level after the cleaning. On that note, I must share that studies have demonstrated that the skin returns itself back to its normal pH of about 5.5 very rapidly anyway- without any help from additional resources.

So my question to you then, is it really necessary to require two cycles of cleansing? Is your skin really that dirty that you need to do this twice daily? Do you live in a coal mine? Unless you do, I tend to think it’s not that important to use this toner and would skip the toner in favor of something else that stays on your skin like an alpha-hydroxy acid.
But for those of you who do like and use the toner, here are the ingredients listed on bottle #2.

Allantoin – A chemical compound naturally produced by many organisms such as animals, plants, and some bacteria. Allatoin is a keratolytic, meaning that it helps to break down the outer layer of dead keratin cells making the skin softer, and allowing (in a very small way) other things to penetrate. It can also moisturize the skin; Aloe barbadensis leaf juice – Commonly called “aloe vera”, it is used because it can reduce the appearance of dryness and flaking. It has also historically been considered an adjunct to wound healing; Anthemis nobilis flower extract – See above; Benzophenone-4 – A safe, FDA approved, over-the-counter sunscreen ingredient. Benzophenone-4 is called Sulisobenzone, and may be used in various concentrations on up to 10%; Glycerin – A sugar-alcohol compound synthesized for manufacturing but can be found naturally. In skin care products it acts as a humectant, and can also be used to thin products; Glycolic acid – A naturally occurring alpha-hydroxy acid which is also synthetically made. When found in unbuffered, fairly significant concentrations, it acts as a mild exfoliant, degreaser, and skin conditioner; Hamamelis virginiana – North American Witch hazel. Used as an astringent; Hydroxyethylcellulose – A polymer of cellulose used as a thickening agent in water-based cosmetics and products; Imidazolidinyl urea – See above; Methylparaben – See above; Panthenol – A form of vitamin B5, it is used as a lubricant and a moisturizer; Polysorbate-20 – An emulsifier for water based products; Propylene glycol – See above; Propylparaben – See above; Rosa canina fruit extract – Skin conditioning agent; Sodium hydroxide – Otherwise known as lye, this ingredient may be used to raise the pH of a product, but is caustic in more concentrated amounts; Sodium PCA – See above; Tetrasodium EDTA – A chelating agent that softens water and acts as a preservative.

Lastly, is the REPAIRING TREATMENT (New PROACTV®).
The main ingredient in this is once again- Benzoyl Peroxide (BPO), 2.5%- the same as what’s in the cleanser. The “New PROACTIV®” has a microfine BPO that is reported to cause less irritation by leaving less clumping, as the larger molecule tends to do.

Benzoyl peroxide is rapidly bactericidal. This means it can kill propionibacterium acnes, (or an easier word), the P.Acnes. P. acnes are the bacteria that is responsible for infectious acne. Benzoyl Peroxide can kill off these types of bacteria often without the use of antibiotics.

It’s important to note that acne actually begins before the P.acnes get involved, and that retinoids are the Gold Standard for Acne therapy. Saying that, if PROACTIV® works for you, and is in your price range, stay on it. But if you are getting buried in payments, you may want to move on to something else to get the main ingredients that I’ve explained above.

Yes,there are benefits of a formed system and routine. And if you need that type of regimen, then PROACTIV® may be the miracle you’re looking for. But if you are strong enough to maintain your own cleaning routine without the guidance, you should be fine with a bit less expensive course of action.

Stem Cell Creams Or Stem Cell Con?

Since “stem-cell” is the buzz phrase of the present, there is no shortage of “entrepreneurs” attempting to cash in on the hype, the mystery, and the promise of this get-rich-quick opportunity. While stem-cell research will likely produce wide-spread benefits for many conditions, the technology is still young.
Now, regarding this stem cell cream do marketers actually expect people to
believe they are the absolutely most important need-to-be cured-right-now wrinkle remedies that contain these all important human stem-cells?
They obviously know their audience. People are buying these creams at a record rate. Unfortunately, the stem cells they are referring to are the stem cells in PLANTS, not human beings. And, for years many creams and ointments have contained ingredients from plants.
Advertising is interesting. The term snake oil has been around forever. Its first recorded use was in China a few hundred years ago. In our country, in the mid 1800′s potions and lotions were sold out of covered wagons….Not so different than being sold today in infomercials!
Getting back to stem cell creams, they may briefly plump up your epidermis (the outermost layer of your skin), making it appear smoother, but have no lasting effect. They also may have light diffusers that make your skin appear brighter. Bottom line is, no real lasting help.

If you are looking for something that WILL work, look to the Retinoid and acid family (such as Retin-A and Glycolic’s). They turn your cells over faster and will make you look younger because your skin actually is younger, and has a solid nutritional source. They are guaranteed to work.
But, some people just cant help being bitten once in a while. If you have already been bitten, dont beat yourself up. We all get bitten once in a while.

Hopefully you didn’t really believe the above headline, but sometimes it seems like all you see are advertisements for cosmetic injections. Many are from medical offices, but if you look at their specialty, they are not even remotely connected with dermatology, skin or the facial structure.
Botox, Dysport, plus a variety of fillers can work miracles, but need to be administered by a person highly skilled and trained as a cosmetic provider. Unfortunately, some medical offices, especially those with a strong base of female patients, see injectables as a great new profit center. Just covert a seldom used room and you’re ready to go.
As a national trainer for the two largest cosmetic pharmaceutical companies, I can tell you its easy to “freeze” the side of a patients face by injecting improperly. Maybe for months. How would you like to have to carry a drool rag with you for the next few months till the effects wear off?
Having a knowledge of the facial muscles is only part of the skills a provider needs. They need to carefully listen and learn what the person receiving the injection wants to achieve. By knowing exactly what the patient expects and wants, you can inject knowing their goal will be accomplished.
Convenience and price are nice things to consider, but they definitely shouldn’t be the most important factors when deciding who you choose to administer any injectable enhancement.

What To Look For In A Cosmetic Medical Provider

1. A good reputation. Reputations are like gold in every medical practice. If a provider is well respected, the community, and your friends, will know. If you are willing to share your plan for cosmetic enhancement with friends, ask them for there recommendations and opinions.
2. One that listens. People seek some form of enhancement, or as many of my patients jokingly say “maintenance”, for individual reasons. Sometimes an outside trigger, like an acquaintance telling you that you look “tired”, creates a desire for “SOMETHING! This is why the medical provider should listen to your personal desires in order to lead you in the right direction, rather than just deliver the BOTOX® you thought you needed and send you on your way.

3. One that lets YOU make the decisions. A good provider will recommend, but still let you be in charge. This is a bit of a continuation of point #2. After listening, they should recommend a treatment or a course of treatments that will get you closest to where you want to be. Then, let you decide how far you want to go based on your choices and your budget.

4. One that isn’t a “Hard Sell Advertiser”. If a medical providers office has a good reputation, they don’t have to hard sell themselves. They already have a strong patient referral base. Some advertising is normal, but if you see ad after ad from one office (think personal injury attorneys) it probably means that they’re not good enough to hang on to patients, much less receive referrals from them. This type of provider survives by hard sell advertising. They need to create a fresh supply constantly instead of relying on reputation to fill their waiting room.

5. One that isn’t priced significantly lower (or higher) than others. If someone has prices significantly higher than the local norm, that may mean they are the best and want to make certain they don’t waste their time seeing someone that can’t afford their services. It may also mean they only want the patient they can overcharge and get away with it. Go back to number one and see what their reputation is in the community.
On the other end of the spectrum are office prices that are “too low”. To me this has always suggested a desperation that announces “we will take anyone! Just come!” Discovering Too Late why their price was so low or their schedule was so flexible can be a crude experience. And one you may live to regret for a long time.

6.One who is knowledgeable about products, treatments and options. Some medical providers try to perform a wide range of services, both injectable and laser, by utilizing only a few different products or treatment procedures. In my office, we offer several fillers and I currently use two different muscle relaxers, depending on Patient Needs. And, I am testing new products all the time.
We also have different lasers with several heads each, depending on your skin type and procedure. Lasers are expensive to purchase, or even lease. If an office spent $70,000 to $100,000 on a laser, they want to see it continue to bring in income as long as possible. Unfortunately, their laser may be outdated. For instance, why subject yourself to more pain than you need for hair removal when a new laser is more comfortable and does a better job? Make sure the equipment in your providers office is state of the art. You would be surprised how fast this technology changes. Four years can mean several generations of improvements.

7. One who can prescribe medication. You might say “no kidding!” but there are multitudes of lower level nurses, estheticians, and “assistants” who take courses and learn the “art of cosmetic improvement”. The problem is this: what if something happens to you that requires medical attention? They are not educated to diagnose a condition that may cause you a great deal of harm. Nor are they able to prescribe anything to help. Often times these folks are “working under a doctor’s license” in order to be able to order the product in the first place. The doctor does not know you, nor perhaps do they even specialize in skin problems. You see this scenario in some medical offices with a large existing female patient base. They want to build a new profit center, so they section off a room or two, and hire an extra person. But they have very little (or no) experience in cosmetic dermotology. Their only real experience in this field has been a short course that was provided by a pharmaceutical company.

8. Know their credentials. Please seek your treatment from one of these providers: Nurse Practitioner (NP), Medical Doctor (MD) Doctor of Osteopathy (DO), or Doctor of Nursing Practice, (DNP). I recently (October of 2011) attended a conference in California with attendees from all over the United States. Some of our discussions were about the states allowing lower level medical personnel (or worse) to inject fillers and muscle relaxers as long as they are in the doctors office. In my state, (Florida) Registered Nurses, who are very well trained and knowledgeable, still aren’t even allowed to turn on a laser. But in some states, almost anyone in the office can perform a laser procedure with very little training. Always check who will be doing the actual treatment.

Quick Note: I did not include Physicians Assistants in the list above because I’m not certain how each state regulates P.A.’s. In our state they are highly respected and more than a “medical assistant” or assistant of the doctor. They have followed a course of study that allows them to diagnose conditions and prescribe medication. To my knowledge though, P.A.’s always have to be affiliated with a physician.

9. One who is a good teacher. Your provider should be able to provide good options for your concerns, and able to teach you about products and procedures. They should also be willing to advise you when something you want may not be in your best interest, and why. In addition, he or she should be able to help you learn how to maintain your desired results longer. Finally, they should be able to direct you to other practitioners (a plastic surgeon for example) if that is what’s needed to obtain the best results.

10. One who has your best interest in mind, not their income. This is somewhat related to point #3. Based on what your provider heard you say, he may recommend a course of treatment that could cost thousands of dollars. They may honestly feel these treatments are the best way to get you what you want. If you have some reservations about the costs, they should also be able to offer something a little less costly. They may have to tell you that you will not end up exactly at the point you want, but at least you will have a choice. I have heard multiple stories of innocent folks coming to an office for “a little work” then leaving with a bill that nearly made them lose consciousness. Guess who benefits from this? ALWAYS be clear if you have any questions BEFORE you allow treatment to be performed.

11. One who is surrounded by knowledgeable and likable staff. It’s an old axiom that happy offices have happy patients. A good staff is sometimes difficult to maintain, but if everyone seems happy (or miserable) every time you go in, there’s probably a good reason. Since it’s your goal to develop a relationship with a good medical provider, their staff should demonstrate they want you to be just as happy as they are.

12. You need a provider that doesn’t look over the top (“bizarre”). This may sound strange, but you have no idea how many people in my field have overdone their appearance. This should not even have to be said, but if your provider comes in looking like a weird, unmovable porcelain doll or has lips that enter the room considerably before she (or he!) does, simply excuse yourself and run.

13. They HAVE to be up to date. We talked about this in #6, but please let me elaborate on being up to date, not only in the equipment they use, but their knowledge. They don’t have to be using everything that was on Oprah last week, but should be aware of the trends in the ever-evolving medical cosmetic industry. That means knowing about, and passing on to you, new products that may be on the horizon. It also means advising you of problems we see such as biofilms, our latest challenge. Biofilms are bacterial colonies residing in the skin that have been found to be associated with “permanent type” fillers. This necessary learning is done through meetings, lectures, and constant study. Here is the final test of a great medical provider: Do some reading yourself, then ask the provider what they think of a particular product. If you find they are somewhat disinterested, they are probably not your best bet. If they become animated and truly enjoy discussing things, you have the beginning of a great relationship. If they offer a couple of other options and briefly tell you the differences, they probably love their job and you have a real winner. Hang on to them!

Basic necessities like food, clean water from our taps, a roof over are head, shoes on our feet, electricity at a flick of a switch are things, I believe we take for granted. You just have to remember a time when the power went out, or your water pipes burst or froze to realize how much we consume everyday. Even in these econmic times our standard of living in the Western developed World has risen condsiderably since the great depression. When people 1920′s 1930′s were lining up to soup kitchens for their only meal of the day.

Tired Of Your Flat Rear End?

Rear end implants are all the rage now, but this is a bit much! And the back story (bad pun)is more bizarre than anyone could ever make up. It seems the owner of this rear end, Oneal Ron Morris, a transgender woman,did this to herself. She has an unusual technique. More on that in a moment. After seeing the results of her handiwork, and thinking it looked pretty good, she allegedly began passing herself off as a plastic surgeon, and performed more “procedures”.

That’s when things began to go wrong for Morris. Some of her “patients” began to show up at Florida hospitals with various parts of their bodies not only way out of proportion, but with signs of infection and open sores.

The problems these ladies were having might have something to do with Ms. Morris technique, mentioned earlier.Instead of using implants and silicone like some old fashioned run of the mill plastic surgeon might use, her method was more on the cutting edge of medical technology.

Mineral oil, super glue and an aerosol can of tire repair, usually called Fix-A-Flat, was her choice. She simply opened a small area and pumped it up. It seems she could buy all she needed for about $20 at the local auto parts store and get up to $700 from her patients for a nice size rear.

The aerosol can of tire repair was especially inventive. Since it contains tetrafluorane to inflate tires, it worked great to inflate body parts. But there is a downside. According to MSDS warnings, it can explode if the temperature gets over 120 degrees. Not the procedure to have just before a trip to the hot mid east to see the great pyramids.

The other downside may of course been her choice of procedure locations. The “Dr.” usually worked out of local motels.

Police finally caught up with “Dr. Morris” in Dade County and charged her with practicing medicine without a license with severe bodily injury.

Police say several people have come forward claiming they were injected by Ms. Morris, many looking just as unusual.

Do Beautiful People Make More Money?

What a non P-C thing to ask! But the answer is yes, according to The Journal of Economic Psychology, University of California. Truth be told, we all know someone getting ahead by on their looks don’t we?

The Journal even went so far as to break it down, concluding that people deemed to be the most attractive make 12% more than their not-so-good-looking co-workers.

That can add up to an easy one million dollars, assuming an average professional salary.

If you’re a hard numbers person (I’m not!), 39% of attractive men and women were judged by appearance only to be more positive, 16% of what were termed middle attractive people created positive reactions, but only 6% of the unattractive people got the thumbs up.

In team exercises, beautiful people seemed to be on the more successful teams. The reasoning: more cooperation on the teams with good looking people, less with the below average group. One theory was that attractive people were more successful in getting others to cooperate.

What’s worse, good looking people start out with advantages over normal folk. Research proves that people will add positive characteristics to attractive people that are not earned. Here’s a thought- Do you think some of our well polished presidential candidates might fit into this “beautiful people” category? Not pointing fingers, but a couple of them look like they just came off a magazine cover. Coupled with a great speech writer and some debate coaching, you have instant presidential material!

Now, I do have a point to make, totally unqualified and un-researched as it may be-
Let’s say a beautiful person was placed on a team with you. Then you find they are (I am searching for a politically correct term here)just plain dumb. Do you think their good looks might work against them and make you dislike them more than a not so good looking dummy? If so, maybe its because you initially placed a higher value on their abilities than you should have. Just a thought.

Here are some tips to help us average people look and be perceived in the best possible light.

” You are a package and clothes are your wrapping. They may not help you perform, but will help how you, and your performance, is perceived.

” Ask a couple of friends what they think is your best physical feature. Emphasize that blessing in your clothing, posture and demeanor.

” Have a trademark: Stand out from the crowd with your own style. I have a friend that only wears white and always looks calm, cool, and collected in the hot South Florida sun. She is in her late 60′s and still turns heads with admiring glances.

” Watch your body language. This is a tough one because we never pay attention to our own. But, we sure look at others, don’t we? Quick tips: NEVER slouch, especially if you are tall. Stand up and be proud of your height. When talking to someone, never look at anyone but them. Make sure your eyes do not dart around or watch others. When stating a point you want remembered, cross your arms. There are a lot of books on body language. Read a couple to pick up some pointers. Most importantly, when you see someone you admire, note their body language and “model” it so well you make it your own.

” Watch your mouth, especially in a business setting. Never use expressions that make you appear less than professional. Proper diction doesn’t goes out of style.

Seven Secrets For Sensational Skin

Knowing these seven secrets will give you the advantage you need to look spectacular.

Secret One: Hands Off! Frequent touching of your skin (especially your face) will leave dirt deposits that not only clog your pores, but can increase dryness by removing moisture barriers. If you have sunscreen on, (you do, don’t you?)when you touch your skin you can also remove that protection.

Secret Two: Always Use Sunscreen, even if you don’t think you will be out in the sun. Remember, you get some incidental rays just by walking to your car. UVA light is constant even in winter months and penetrates much deeper than UVB (the burning rays). UVA damage is responsible for most skin “aging”.

Secret Three: Drink plenty of water. Even though water does not directly translate into hydrated skin, it helps your body and skin remain healthy. If you become dehydrated, it can be visible in your skin, your eyes, and even your tongue. Water helps everything in your body to work properly. Please don’t skimp.

Secret Four: Cleanse skin twice daily. If twice is out of the question, at least cleanse you face in the morning, after your body has gone through the night-time regeneration process. Cleansing your face at night before bedtime has the advantage of an extra cleaning, and a reminder to use whatever product your practitioner recommends. If you use a retinoid (highly recommended unless you are pregnant) this is when it goes on.

Secret Five: Change your sheets at least weekly. It may sound silly, but not everyone changes that often. If your pillow case isn’t clean, your face will re-absorb much of the nasty residue shed by your skin as you toss and turn at night. Keeping linens clean is healthy for your skin.

Secret Six: Routinely clean your make-up brushes. I have patients who keep the same brushes for years, and never clean them! While it doesn’t need to be done every day, it should be done at least once a month. If you loan a brush to a friend, go ahead and clean it right away. You never know….

Secret Seven: This may be the most important secret: Forget About Your Skin! From my perspective, the damage caused by folks obsessing over their skin is huge. If you get a pimple, leave it alone. Spot treating can cause dryness or irritation, and picking may very well result in a scar. When is it OK to be concerned? When you have a “pimple” that has lasted for months, or a “dry patch” that just doesn’t go away. These problems are reserved for your dermatology office.

Recently I talked about the FDA’s future recommendations for sunscreen makers. But preaching sunscreen does nothing if you’re dying to look tan. It seems some people get “addicted” to the feeling of sun on their skin, and relish the change in color. It’s hard to believe, but they actually become “addicted” to making their appearance change. Unfortunately, these changes are almost always unattractive….You look weird…And you can permanently damage your body.

It seems appropriate here to mention Patricia Krentcil, the 44 year old “super-tan mom” from New Jersey. In May she was arrested for allegedly bringing her daughter into a tanning booth. True or not, a bigger issue is the apparent addiction this woman has to tanning.  I found a video on you tube that says it far better than I ever could. It’s only just over 2 minutes long, but it made me cringe just watching.

Here is a photo of Patricia when she was younger. A beautiful, vivacious looking lady, probably between 19 and 25. Incredible…

While beauty is in the eye of the beholder, I doubt there are many beholders who would find her tan beautiful. Or even safe. At least I hope not!

Unfortunately,  while Patricia Krentcil may be “tanorexic”, being “over-done” is not reserved for just the tanning booth, no pun intended. In today’s world I see lips that would put the average duck to shame and cheeks so big and puffy they need a bra. Yet most of these people (victims?) cant see their own extremes. As a practitioner my question is- “why would a medical professional do this to them”?

To the patient I would say the answer is two-fold; they forget what they looked like at their best, then become hooked on the idea of change.

To the provider I would say they need to get an objective opinion of their own appearance first. I have noticed that “over-the-top” providers seem to spawn over over-the-top patients. Not all, but many.

Then, somewhat sadly, there’s the “elephant in the room” nobody wants to mention… The provider treats because the patient is willing to pay. I wont belabor this other than to say that I, and most in the medical profession, will decline rather than have a patient leave with a treatment they would be ashamed to admit was their work.

Even those of us who want to look better but are more conservative should keep a favorite picture for reality checks. If the picture was many years ago, consider what about the picture makes you most attractive. Also take note of how natural and relaxed you look. Thats the look you want to preserve.

One final note. Providers welcome you bringing in a pleasing photo and your comments about what you find most attractive. That can start a great dialogue which will give you a happy result.
